9. SMS Messaging Terms
  • SMS Messages are sent by Advanced Physical Medicine in the form of appointment reminders, confirmation messages, marketing messages, follow-up messages, customer service messages, and similar communications after opting in.
  • You can cancel the SMS service at any time. Just text “STOP” to our short code. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time, or text START and we will start sending SMS messages to you again.
  • If you are experiencing issues with the messaging program you can reply with the keyword HELP for more assistance, or you can get help directly at officemanager@advancedphysicalmedicine.com or by calling 407-282-3615.
  • Carriers are not liable for delayed or undelivered messages

Confidentiality

Client records are regarded as confidential and therefore will not be divulged to any third party, other if legally required to do so to the appropriate authorities. Clients have the right to request sight of, and copies of any and all Client Records we keep, on the proviso that we are given reasonable notice of such a request. Clients are requested to retain copies of any literature issued in relation to the provision of our services. Where appropriate, we shall issue Clients with appropriate written information, handouts or copies of records as part of an agreed contract, for the benefit of both parties.
